OMIG Posts Updated Mandatory Compliance Certification Form

OMIG has posted on its website the updated Mandatory Compliance Certification form for January - November 2015. The form is available at: http://www.omig.ny.gov/compliance/certification

***Those providers who completed the certification process in December 2014 do not need to recertify at this time***

Note, this form has been updated to primarily address the certification obligation for newly enrolling providers as well as current Medicaid providers whose enrollment is being revalidated by the New York State Department of Health.
